ABOUT Médecins Sans Frontières (MSF) is a non-profit, independent international humanitarian organization committed to delivering medical aid to populations facing crises, with a steadfast commitment to impartiality and without regard to race, religion, creed, or political affiliation. In Malawi, MSF collaborates closely with the Ministry of Health and the local community, particularly in the Cervical Cancer project in the Blantyre and Chiradzulu districts, as well as the CBO Project in Zalewa and Dedza District.
Malawi has the highest cervical cancer incidence (76/100,000) and mortality rate (50/100,000) in the world, representing an estimated 3,684 new cases and 2,314 deaths annually. Eastern and Southern Africa, including Malawi, also has one of the highest HIV prevalence, which contributes significantly to the cancer incidence, as HIV positive women are significantly more likely to develop cervical cancer when infected with HPV. Despite ongoing efforts to provide cancer screening and treatment services by the Ministry of Health and international organizations, care coverage remains low in comparison to the scale of need in the country.
MSF, in response to this need, is implementing a unique comprehensive oncology project in Malawi. The project aims to encompass all components of gynecologic oncology care (mostly cervical cancers) from primary to tertiary prevention through the collaborative work of a multidisciplinary medical team embedded at the Queen Elizabeth Central Hospital (QECH) and community health centers in the Blantyre and Chiradzulu districts in Malawi.

MEDICAL DOCTOR 4B – 1 POSITION

Main Purpose
Provide outpatient and/or inpatient medical care to patients/beneficiaries according to adapted and updated medical knowledge, MSF protocols, values and universal hygiene standards to improve the patients/beneficiaries health conditions.
Main Responsibilities
Providing safe, evidence-based medical care to oncology patients in both OPD and IPD. Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting protocols. Keep the patient and/or his/her family informed about the illness and provide appropriate explanations about the treatment to follow, checking they have understood. Follow up the evolution of the hospitalized/IPD patients, through daily visits, consultations and examinations, prescribing the necessary treatment following protocols, deciding whether they can be discharged or transferred to other departments – in collaboration with other doctors- and informing their family about the patient’s Check and control the rational use of medicines and equipment under his/her responsibility Contributing to quality improvement, audit activities, and morbidity/mortality reviews. Ensure ongoing training of the medical/paramedical multidisciplinary team to optimize the quality of care. Knows and ensures medical protocols are followed and implemented, checking universal precautions are always followed and reducing bio-hazard risks and improving infection control. Ensuring appropriate pre-chemotherapy assessment and timely identification of contraindications or complications. Prompt recognition and management of chemotherapy-related adverse effects and oncologic emergencies. Adhering to chemotherapy protocols, infection prevention measures, and medication safety standards. Maintaining accurate, complete, and timely clinical documentation. Protecting patient confidentiality and upholding medical ethics. Supporting teamwork and respectful collaboration within the multidisciplinary oncology team. Specific Accountabilities
A. Clinical Care – OPD (Chemotherapy & Oncology Clinic)
Conduct pre-chemotherapy clinical assessments (history, physical examination, performance status, Review laboratory and imaging results prior to chemotherapy administration). Identify contraindications or reasons to delay treatment and discuss with the Oncologist. Manage common chemotherapy side effects (e.g., nausea, vomiting, mucositis, diarrhea, neutropenia, anemia, fatigue). Provide supportive and symptomatic treatment. Educate patients and caregivers on treatment plans, side effects, and warning signs. Ensure proper documentation in patient files and electronic systems (if applicable). Participate in multidisciplinary case discussions when required. B. Clinical Care – IPD (Hospitalized Oncology Patients)
Provide daily medical review of admitted oncology and chemotherapy patients. Manage chemotherapy-related complications (e.g., febrile neutropenia, dehydration, infection, bleeding, severe GI toxicity). Performs doctor on call duties in the Oncology Department. Initiate emergency management and escalate complex cases to the Oncologist. Coordinate referrals to ICU, surgery, radiology, or other specialties when needed. Ensure safe prescribing practices and medication reconciliation. Support palliative and end-of-life care management in collaboration with the team. C. Chemotherapy Safety & Quality of Care
Ensure adherence to chemotherapy protocols and safety standards. Verify chemotherapy orders with the Oncologist before administration. Monitor patients during and after chemotherapy infusions when required. Report and document adverse drug reactions and incidents. Contribute to quality improvement initiatives within the unit. D. Collaboration & Teamwork
Work closely with the Oncologist in treatment planning and patient follow-up. Collaborate with oncology nurses for patient monitoring and education. Maintain collaboration between 4C Gynecological Oncology and 4B Oncology Departments. Coordinate with pharmacy for chemotherapy preparation and drug availability. Participate in multidisciplinary meetings and mortality/morbidity reviews. Support mentoring and capacity building of junior staff. E. Administrative & Reporting Duties
Maintain accurate and timely clinical documentation. Contribute to monthly activity reports and clinical data collection. Support audit activities and protocol reviews. Assist in developing and updating SOPs related to chemotherapy care. Requirements
Education Candidate must be a holder of an MBBS or equivalent. Valid registration with Medical Council of Malawi Experience At least 2 years of clinical experience (experience in oncology is an asset Training or experience in chemotherapy management is desirable Experience in managing critically ill patients is an advantage Language Fluency in English and Chichewa Knowledge Essential computer literacy (Word, Excel and Outlook) Competencies Results and Quality orientation Teamwork and Cooperation Behavioural Flexibility Commitment to MSF principles Stress Management How to apply
